Adding new Product Hierarchy causes error

Adding a hierarchy item through the Products module displays an error after I click the submit button. The message merely says "error = Array" (no quotes). I can add products, fields, categories, and any type of template, but I can't add hierarchy items. I upgraded the cmsms core and all modules thinking it might solve the issue. When it didn't, I uninstalled the Products module, removed a 'cms_module_products_categories' table that was left over in the database, and reinstalled Products without success. I've used the Products module with hierarchies on previous websites and had great success. Could someone please help me troubleshoot this issue?

Following up with this...
I decided to try a fresh installation as opposed to an upgrade. After installing the base version of CMSMS 1.6.6 with the latest Products, CGSimpleSmarty, and CGExtensions I'm still getting that ambiguous "error = Array" when I try to create a hierarchy. Am I missing something or is this a new bug?

I ended up adding the hierarchy items directly through the database. They appear just fine but I get the same error when I try to edit them.

I'm actually recieving a couple more errors:

DEBUG2: SITENAME
DEBUG3: SITENAME
DEBUG: setting color transparent
error = Array

Help would be very much appreaciated
