'Filelist' and 'file_list' (which is what we use) are tags, not modules. You install tags by uploading the file(s) to the 'plugins' folder (via your FTP program). Go to Modules Manager, Available Modules, "F" and toward the bottom, just under FormBuilder, you should see Forum v 0.9.2. It works very nicely.

No, just added the "File Upload" field to the form - no tie-in with the "Uploads Module". It worked so well, didn't see the need to complicate things by integrating with another module!
